A Personal Approach
Christian Horizons is committed to providing person-centred supports. We help people define and pursue their own goals for personal growth. We believe everyone has the right to experience inclusion and enjoy relationships and meaningful activities. We strive to keep the person with developmental disabilities, as well as their family and friends, at the centre of planning and decision-making for that person.
How to Access Services
Christian Horizons works with agencies in local communities to identify needs and provide supports. Our ability to support a person is determined on an individual basis and depends on whether a person’s needs can be met with available resources.
Individual Support & Respite Care
Individual Support Services offer opportunities for in-home respite care, out-of-home respite care, transportation, school-to-work transitional supports, and other alternatives to meet the unique needs of individuals and their families.
Community Residences
Community residences provide a quality residential setting for adults and children who require assistance in daily living. In this type of setting, three to six individuals live in shared accommodations and usually receive 24-hour daily support by qualified staff.
Supported Independent Living
This is an option for a person who chooses to live independently or with a roommate. The level of support is determined on an individual basis and can range from several hours a day to a few hours a week.
Family Home Services
If individuals prefer a family home setting, they may be supported in a family home. In this model, the person shares the family’s home, similar to foster care. Individuals are matched with families carefully to ensure compatibility for the long-term. A Christian Horizons staff member meets with the family home provider and the supported individual regularly to offer guidance.
Community Participation & Career Development Supports
Christian Horizons encourages the people we support to pursue volunteer, recreational and employment opportunities. Our community support services help develop life skills, nurture community involvement and vocational and recreational activities. These supports are explored on an individual basis, and help enable the person to participate in meaningful and interesting pursuits.
