Individual Support Services
Individual Support Services offer opportunities for in-home respite, out-of-home respite, transportation, school-to-work transitional supports, and other alternatives to meet the unique needs of individuals and their families.
Supported Independent Living

This model of service is an option for a person who chooses to live independently or with a roommate. The level of support is determined on an individual basis and can range from several hours per day to a few hours each week.
Day Supports | Community Support Services
People supported by Christian Horizons are encouraged to pursue employment, volunteer, leisure and recreation opportunities. Our community support services offer opportunities for life skill development, community involvement, vocational and recreational activities. These are explored on an individual basis, and supports are available to enable the person to participate in meaningful and interesting activities.
Family Home Services
Where a person prefers a family-like setting, he/she may be supported in a Family Home. In this support model, the person shares the family home with a family, similar to foster care. The matching of individuals with families is done carefully and sensitively, to ensure compatibility and a long-term arrangement. A staff from Christian Horizons frequently meets with the family home provider and the individual to offer support and guidance.
Community Residences
Community residences provide a quality residential setting for adults and children who require assistance in daily living. In this type of setting, 3 to 6 individuals live in shared accommodations and usually receive 24-hours/day support by qualified support staff.
